Information you provide: name, phone number, email address, general care descriptions, and any other information you voluntarily submit through our consultation-request, contractor-application, or referral forms.
Information collected automatically: standard web server logs including IP address, browser type, referring page, and timestamps. We may use minimal first-party cookies required for site functionality. We do not use third-party advertising trackers.
How We Use Your Information
To respond to inquiries, place referrals, process contractor applications, maintain compliance records required by AHCA, prevent fraud and abuse, and improve the website. We do not sell personal information.
Sharing
Form submissions are transmitted to our business email. We may share information with: our email/form-processing vendor (operating under a business-associate agreement where PHI is involved), the independent-contractor nurses you are matched with, professional advisors, and state and federal regulators (such as AHCA) when required by law.
Retention & Security
We retain records for the longer of seven (7) years or the period required by AHCA and Fla. Admin. Code 59A-18. We use reasonable administrative, physical, and technical safeguards to protect the information we hold, but no internet transmission can be guaranteed 100% secure.
Your Choices
You may request access to, correction of, or deletion of information you have submitted by emailing info@opuluxenursing.com. Requests are subject to regulatory records-retention obligations.
Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children. If you believe a child has submitted information, please contact us and we will delete it.
